Purpose:
To outline required information a requestor must submit to the Kansas State University Veterinary Medical Teaching Hospital (KSU-VMTH) to obtain VMDB data.

Requestor must submit an e-mail request containing the following information:
· Name, title, and affiliation with your institution
· Purpose of request
· Name of Study/Project
· If for possible publication/presentations, list all individuals involved
· List specifics of request:
o Medical record number
o Date of birth
o Date of service
o Specific data to be copied
The fee for record copies is 50 cents per page. Pre-payment is required. There will be no fee if a KSU-VMTH faculty member is involved in the study.
The contents of the record that can be copied and forwarded to the requestor are limited to:
o Case Summary
o Physical Examination/Assessment
o Diagnostic Reports
o Prescription Log
o Surgery Report
o Discharge Instructions
Copies of records will exclude client and referring veterinarian data.
If additional information is needed from documents not copied, a KSU-VMTH faculty member must be included in the study. The faculty member will collate or summarize the additional information needed and forward a summary to the investigator.
A KSU-VMTH faculty member must be included as co-author, or otherwise acknowledged, when data is used in publications and/or presentations.
