Submit requests detailing the following information:

· purpose

· name and title of person requesting the information

· names and titles of other people involved in the study

· name of the study/project

· specific data requested

There will be a fee for copying the records, (price available upon request).  Identifying information pertaining to the client, patient and/or referring veterinarian will be removed.

The Ontario Veterinary College will need to be acknowledged if the data is used in any publications and/or presentations.
