• List any other people involved if for possible publication/presentations
• Name of Study/Project
• Specific data requested, include MR#s (e.g., DOB, copies of history, etc.)

There may be a fee of 25 cents per page if copying records.  

Copies of records will exclude client and referral veterinarian identifying data.   It is possible that copies of client communications and accounting information may not be reproduced

A faculty member from the Department of Veterinary Clinical Medicine may need to be included as a co-author, or otherwise acknowledged in publications or presentations arising from information provided from clinical records.
