· List any other people involved if for possible publication/presentations
· Name of Study/Project
· Specific data requested, include MR#s (e.g., DOB, copies of history, etc.)
There may be a fee of 25 cents per page if copying records.
Copies of records will exclude client and referral veterinarian identifying data. It is possible that copies of client communications and accounting information may not be reproduced.
A University of Missouri Veterinary Medical Teaching Hospital faculty member may need to be included as a co-author, or otherwise acknowledged, when data is used in publications and/or presentations.
